In the top picture you can see that the splines have been worn down. This is usually caused by not fitting the shaft in correctly or not getting the mating coupling lined up right.
This shaft most certainly will need to be replaced in order for your unit to work properly.

The middle picture that this input taper shaft is fine to use again. It doesn’t have any burrs,nicks,deep scratches or any wear or tear. It is smooth to the touch and has no contamination on it of any kind.

The bottom picture shows a fractured taper. Again, if it is not fitted in properly or not matching up to the mating coupling or even the amount of torque can cause such damage.
Following manufactured procedures for all these causes, can be avoided.
